Beneficiary Forms

Make sure that you have current beneficiary forms in your employees HR files. Your employees may have event changes (marriage, divorce, births, etc.) and they forget about how this may change their beneficiary designations.

We suggest that you regularly remind your employees to notify you if they need to change their beneficiaries or consider giving new forms to all of your employees annually.
